Consent to Receive SMS Communications
By providing your mobile phone number to Whole Health Center (“WHC”), you consent to receive SMS/text messages regarding your appointments and other practice-related notifications. These messages may be sent via an automated system.
Types of Messages You May Receive:
Appointment reminders and scheduling confirmations
Intake paperwork or form completion requests
Occasional practice updates, surveys, or promotions (if applicable in the future)
Message Frequency:
Message frequency will vary based on your appointment schedule and engagement with our services.
Message and Data Rates:
Standard message and data rates may apply, depending on your mobile carrier plan. WHC is not responsible for any charges incurred.
Carrier Disclaimer
Mobile carriers are not liable for delayed or undelivered messages.
Two-Way Communication:
While WHC primarily sends one-way SMS notifications, replies such as “STOP” or “HELP” are supported. We do not currently support conversational or real-time SMS communication.
Opting Out:
You can opt out of SMS messages at any time by replying “STOP” to any message. After opting out, you will no longer receive SMS communications but may still receive emails or phone calls where appropriate.

Data Collection and Usage:
Your phone number and any SMS communication data are stored securely and used only for the purposes of message delivery and account management. Your information will not be shared with third parties or used for marketing without your consent.
No Condition of Service:
SMS consent is not required as a condition to receive healthcare services, unless appointment reminders are deemed necessary for the proper coordination of care.
Scope of Consent
Your consent applies only to the phone number you provided. If your number changes, you must notify WHC to update your consent.
HIPAA & Security Notice
While WHC takes steps to protect your information, SMS/text messages are not a secure communication method and should not be used to transmit personal health information (PHI). By consenting, you acknowledge and accept this risk.
Privacy & Security:
Although WHC implements safeguards to protect your information, SMS is not a fully secure method of communication. By consenting to SMS, you acknowledge and accept this risk.
